Visit the Commission's ALiS Licensing System and log in with your account credentials.

If this is your laboratory or entity's first time, you'll need to create an account first — it only takes a minute.
Create an account ↗Once you're logged in, look at the left-hand menu. Select:
- Laboratory Application for Changes/Updates
This option covers all three of the following submission types:
- Proficiency Monitoring Forms (initial and updates)
- Licensee Change Reporting Forms
- De minimis Laboratory Updates (interpreting and non-interpretive licensees)
Follow the on-screen instructions to complete your application. Fill in all required fields before proceeding.
Click Submit. A document upload screen will appear immediately after.
- Upload all relevant supporting documents on this screen.
- Do not navigate away before uploading — the upload screen appears only once per submission.
Once received, the Commission will review your submission and follow up if anything additional is needed.

If this is your first time, create an account before logging in.
Create an account ↗In the left-hand menu, select CFE Approval Request.
Follow the on-screen instructions to complete all required fields, including:
- Course title, date, and format
- Presenter information
- Course description and learning objectives
Once all required fields are complete, upload your course information and agenda, then submit the application to the Commission.
You will receive a confirmation from the Commission once your CFE course is approved. Allow adequate lead time before the scheduled course date.
